The following insurance companies are in Network: Regence, ODS, MHN, Aetna, United Behavioral Health and Pacificare, and Providence Preferred. Please note that many health insurance agencies contract with different companies for mental health benefits. Please check with your primary insurer to verify who provides does your coverage. We will help verify mental health benefits prior to your first visit so you always know the costs in advance. If your insurance company gets mental health benefits through a company not listed above, you may be eligible for out of network benefits. If so we can provide the in-

formation needed to submit for reimbursement.

In addition it may be helpful to bring a few notes about the reason for coming and some background information on the forms provided. If you have copies of any prior evaluations or psychological testing, please bring copies also. A release of information will be completed when we meet prior to my contacting anyone on your behalf.
